چکیده

Objectives : To retrospectively study and compare the clinical outcome of the patients undergoing anterior cervical discectomy (ACD), anterior cervical discectomy with fusion (ACDF), or anterior cervical discectomy with fusion and instrumentation (ACDFI) for cervical spondylosis. Methods : A retrospective analysis of 80 patients with cervical spondylosis who had undergone anterior cervical discectomy with or without fusion according to our preoperative assessment, were undertaken. Clinical outcome and associated complications in each group were analyzed to determine the need and efficacy of each procedure in these patients. Results : ACD and ACDF group had almost similar results in terms of hospital stay, mean time for improvement and patient satisfaction. ACDFI group had more prolonged hospital stay, took longer time to recover and to return to maximum normal status because of their poorer preoperative neurological status. Conclusions : Fusion is not routinely required, but it is indicated in patients with multisegmental disease, instability, loss of cervical lordosis, hard disc and osteophytic bars. Keys to achieve desirable clinical results are proper patient selection and adequate surgical decompression. No single gold standard procedure exists for the treatment of patients with cervical spondylosis and the treatment strategies need to be tailored to match the specific changes present in a given patient.

Comparison of outcomes and safety of using hydroxyapatite granules as a substitute for autograft in cervical cages for anterior cervical discectomy and interbody fusion

Background:   After cervical discectomy, autogenetic bone is packed into the cage to increase the rate of union between adjacent vertebral bodies, but donor site–related complications can still occur. In this study we evaluate the use   of hydroxyapatite granules as a substitute for autograft for interbody fusion.     Methods:   From November 2008 to November 2011, 236 patients participated in ...

Biomechanical comparison between anterior cervical discectomy with fusion and without fusion

The aim of this study is to compare patients undergoing one level anterior cervical discectomy without fusion versus anterior cervical discectomy with fusion. The study included forty-eight patients operated at either C5-C6 level or at the C6-C7 level: a group of anterior cervical microdiscectomy without fusion performed at one level on 24 consecutive patients was matched to a second group of 2...

Surgery in cervical disc herniation: anterior cervical discectomy without fusion or with fusion

The study included a group of anterior cervical microdiscectomy without fusion performed at one level (either C5-C6 level or at the C6-C7 level) and a second group of patients with same single-level of anterior cervical discectomy with fusion. The kinematic analysis included the range of motion, anteroposterior translation and disc height assessed for the cervical functional spinal units at the...

THE EFFECTS OF ANTERIOR DISCECTOMY AND INTERPOSITION AL IMPLANT UPON LUMBAR MOTION SEGMENT STABILITY

The cadaver spine motion segment behavior under torsional load was evaluated with the disc intact, with partial anterior discectomy and with spacer insertion. The results of this study explain how anterior lumbar discectomy and interbody fusion (ALIF) affects the torsional stability of the motion segment. The pseudarthrosis rate of the anterior lumbar discectomy and interbody fusion (ALIF)...
